The last link is a PDF but may not be directly related to SQL Server. It's database related in general. It could be an intellectual property but you might give it a read to get an idea.

The first link has some in-depth explanation. But again, it may not be exactly what you were looking for but should get you a good starting point. 

You might want to check out other links as well just to help you prepare yours.

What you are asking is bit out of scope for this forum as SOP may different for different environment. Also what points you need in your SOP. I believe if you sit down and get various points from links Mohsin posted you would be able to design your own.
