A web-based tool in Microsoft 365 that enables users to quickly create surveys, quizzes, polls, and feedback forms.
The session ID and correlation ID shown in the error are internal diagnostics identifiers. They do not themselves explain or fix the problem. They are used by the service owner or administrator to trace what went wrong at the time of the request.
For this issue:
- Capture the full error message shown on the page along with:
- Session ID:

e2cd18b1-9ff4-4293-926f-ac9a9d090272 - Date and time (including time zone) when the error occurred.
- Session ID:
- Contact the owner/admin of the Microsoft Form or the organization’s IT/helpdesk and provide these details. The admin can:
- Open the Unified Logging System (ULS) or equivalent service logs for the time of the error.
- Search for the correlation ID to trace the exact request and see what failed.
- If the form is part of a SharePoint/Forms solution managed by an organization, the admin may need to check:
- Whether the form or underlying SharePoint resource is still available and shared with the account being used.
- Any recent configuration or policy changes that might block access.
The correlation ID is unique per request and is only useful to the service’s admin in combination with server logs; it is not an error code that can be looked up or fixed directly by an end user.
