Client host [*.*.*.*] blocked using Spamhaus

I just started getting this message today when trying to send emails to external clients:

*******.mail.protection.outlook.com gave this error:
Client host [*.*.*.*] blocked using Spamhaus. To request removal from this list see http://www.spamhaus.org/lookup.lasso (S3130). [******.eop-EUR03.prod.protection.outlook.com]

Your message wasn't delivered due to a permission or security issue. It may have been rejected by a moderator, the address may only accept email from certain senders, or another restriction may be preventing delivery.

My email server has no viruses.  No changes have been made to it in months.

When checking my IP at Spamhaus, it says it comes up once in the XLB because it is listed in the CBL.  I requested that it be removed but the issue persists.

The issue will persist until you're removed.

The removal requests work but is still reoccurring. 

What settings in exchange could affect this?

I've added a TXT record with the external domain provider for the OWA URL and a PTR with the ISP so hopefully that will be enough?

Spamhaus/CBL finally responded to my email with more information.  

This might be indicating it was my firewall (external device) email reports that might have been triggering the block, not the exchange server. I had it sending emails directly and I guess spamhaus didn't like that.  I changed some settings around.  Now to see if it happens again.
