Hi Mito,
>>But,how to bring the FSW back?
1.By restarting server.
2.By recreating
Find out the reason why FSW fail, repair the issue and it would work again.
Best Regards,
Leo

Hi,
In Windows 2008 the cluster can survive without Quorum also
In Windows 2012 R2 there is option of Dynamic Quorum

Hi
Firstly The Quorum disk should have multiple connections to the Storage and MPIO should be configured. Incase the disk goes offline please refer to the article which states how to start SQL cluster without Quorum. This may work for File cluster as well, however I could not test the same so please test once before implementation
