Am getting this randomly on Windows 10 1709 machines along with the following entry in the event viewer. Some machines are doing it regually, some never, and some every now and again. Same image, same model of PC.

Hi,
According to your description, I suggest you to perform system file checker. Follow the steps below which will help you:
1. Press Windows key + X, click Command Prompt (Admin).
2. In the Command Prompt, type the following command, and then press ENTER:
sfc /scannow
You can follow the steps in the following article:
If the issue persists, perform a clean boot and check which third party is causing this issue.

No System File Check runs fine, it does it on a blank windows 10 1709 machine, think one of our policies is doing it, perhaps the one that turns cortana off, but the error is random.

Hi,
I would suggest you to try the below methods and check if that helps:
Method 1: Restart Windows Explorer
1. Press Ctrl+Alt+Delete from your Keyboard and then click on Task Manager.
2. Now find Windows Explorer from process list of windows task manager.
3. Right click on Windows Explorer & select Restart.
Method 2: Run the Search and Indexing Troubleshooter.
1. Open Control Panel and select Troubleshooting.
2. Click on Search and Indexing Troubleshooter.
3. Run the Troubleshooter.
Method 3: Run the DISM Tool.
If the issue persists, I would have you run the DISM tool to check the system health and will try to restore the files.
Please open Command Prompt as administrator and run the following command:
DISM.exe /Online /Cleanup-image /Restorehealth
Note: The tool might take 15-20 minutes to finish running, so please do not cancel it.
This method may work for you to fix windows 10 start menu & cortana not working.
