Ive recently been seeing this in the wsyncmgr.log.....
Deleting \?\UNC\xxxxxx\SCCM Update Deployment Packages\Baseline - Windows Update - 10-04-2018 - Server 2008\22789445-cd5c-417c-80d8-532157dde55e, FAILED, Win32 Error = 145 SMS_WSUS_SYNC_MANAGER 28/10/2019 17:56:08 3112 (0x0C28)
Failed to delete orphaned content folder \xxxxxxx\SCCM Update Deployment Packages\Baseline - Windows Update - 10-04-2018 - Server 2008\22789445-cd5c-417c-80d8-532157dde55e, error 0x91 SMS_WSUS_SYNC_MANAGER 28/10/2019 17:56:08 3112 (0x0C28)
Now error 145 or 0x91 seem to suggest this means "folder not empty" (could be totally wrong here), I am at a loss as to what the next step is to fix this issue! The Primary site server has full access rights to the folder structure.
This error is literally filling up the log file with hundreds of entries.. over and over!

145 = "The directory is not empty."
Have you manually examined the locations specified to see if they are or are not empty?

Yes, all the folders have a cab file in each.
Are you saying the only way around this is to manually go through the list and get rid?
Id love to know what caused it in the first place!

Have you tried deleting the the files manually?
My guess is that something else has the file locked. The usual culprit here is your AV.

As Jason said , delete the folders manually and run WSUS cleanup wizard

Hi,
Deleted the files in each content folder manually, re-ran the sync a few times, and during all that, ran the WSUS DB optimise scripts, plus enabled the three 1906 options for WSUS Maintenance, and all seems well so far!
Thanks for the help and advice.
It still sits there in the logs for about 1hr doing "Calling ImportUpdateFromCatalogSite for driver update GUIDs" whatever that actually is, but seems to work ok after that.

As noted, the files were most likely locked by something at the time the site was trying to delete them and by the time you got to them, they weren't locked anymore.
DB optimize has no impact here.

Glad the issue has been solved. To make our thread clear to read, here is a short summary for it. Problem/Symptom:
Unable to delete orphaned content in SCCM and see the error 145 and 0x91 in wsyncmgr.log.
Possible Cause:
As Jason mentioned, the files were most likely locked by something at the time the site was trying to delete them.
Solution:
Deleted the files in each content folder manually, re-ran the sync a few times, and during all that, ran the WSUS DB optimise scripts, plus enabled the three 1906 options for WSUS Maintenance.
