Azure OpenAI and Azure AI Speech in UAE North: is all processing and storage guaranteed inside the UAE for a healthcare assistant?

Hello,

I'm a surgeon in the UAE and I'm evaluating Azure before subscribing. I plan to build a clinical AI assistant that handles patient data: transcribing recorded consultations, reading lab and radiology reports, suggesting differential diagnoses, checking medications against comorbidities, and talking with patients by voice. By UAE health-data rules, all patient data must be stored AND processed inside the UAE.

My questions:

  1. If I use Azure OpenAI with a Regional (not Global or Data Zone) deployment in UAE North, does all inference stay inside the UAE? Do prompts, completions, logs or abuse-monitoring data ever leave the UAE? Can abuse monitoring or human review be turned off for healthcare?
  2. Which models are currently GA for Regional deployment in UAE North (for example gpt-6-sol Regional Provisioned)? Is gpt-6.1 planned for UAE North, and is there a Standard (pay-per-token) regional option, or only Provisioned? What is the minimum PTU commitment?
  3. Voice: is any real-time speech-to-speech option (Realtime API or Voice Live) processed inside the UAE, or only Azure AI Speech speech-to-text and text-to-speech in UAE North?
  4. Does Azure AI Speech in UAE North offer Arabic neural voices (Gulf or Levantine dialect)? If I create a custom neural voice (my own voice, with consent), where is it trained and stored, and can it be hosted and run in UAE North?
  5. Which compliance documents cover UAE health-data rules (DHA, the UAE health data law), and where can I find the DPA and the sub-processor list for UAE regions?

Thank you.

Do not process patient data until Microsoft provides written, architecture-specific confirmation that every selected component—including abuse monitoring, support access, telemetry, custom-voice training, and disaster recovery—meets your UAE-only processing and storage requirement.

    Azure OpenAI residency and monitoring

A Regional deployment is the appropriate deployment type: Microsoft’s availability documentation says Standard/Regional and Regional Provisioned inference is processed in the deployment’s region. Global deployments may process worldwide, and Data Zone deployments within their specified zone.^1^

However, the privacy documentation uses the broader commitment that non-Global/non-Data-Zone prompts and responses are processed within the customer-selected geography and may move between regions in that geography for operational purposes. It also says:

    • Models are stateless; prompts and completions are not stored in the model or used to train base models.
    • Stateful features such as Responses, Assistants Threads, Files, vector stores, and stored completions persist data in the resource’s geography.
    • Flagged prompts/completions can be stored in a geography-specific abuse-monitoring store and accessed by authorized Microsoft reviewers. Only EEA deployments explicitly state that reviewers are located in the EEA.^2^

Consequently, the public documentation alone does not establish a complete UAE-country-only guarantee for all ancillary operations.

For sensitive healthcare workloads, apply for modified abuse monitoring before production use. Approval is limited-access eligibility—not an automatic healthcare setting. If approved, Microsoft does not perform the described prompt/completion storage and human-review process, although automated real-time review can remain.^3^

    Current UAE North model options

As of October 23, 2026, the UAE North Regional Provisioned Managed table lists:

    • gpt-4.1 2025-04-14
    • gpt-4o 2024-05-13, 2024-08-06, and 2024-11-20
    • gpt-5-mini 2025-08-07
    • gpt-5.1 2025-11-13
    • gpt-6-sol 2026-09-22
    • o1 2024-12-17
    • o3-mini 2025-01-31
    • o4-mini 2025-04-16^1^

gpt-6.1-sol 2026-09-29 appears for UAE North under Global Standard and Global Provisioned, but not in the supplied Regional Provisioned UAE North table. A UAE North resource under a Global SKU does not mean UAE-local inference. “No access request needed” describes access eligibility, not regional deployment availability.^4^

The supplied availability material does not establish a UAE North Regional Standard/pay-per-token SKU. Verify the Regional Standard pivot or deployment selector before purchasing; do not infer it from Global Standard availability.

For Regional Provisioned, the documented minimum is 50 PTUs, in 50-PTU increments, for gpt-6-sol. The general sizing table also gives 50 PTUs for gpt-6.1-sol, but that becomes relevant only if Regional Provisioned availability in UAE North is independently confirmed.^5^

    Voice and Speech

For strict UAE processing, use a UAE North Azure Speech resource with standalone Speech features:

    • Real-time and batch speech-to-text
    • Neural text-to-speech and batch synthesis
    • Speech translation
    • Custom-voice hosting, but not custom-voice training

Azure Speech states that ordinary Speech data is stored and processed in the resource region. Voice Live is an exception: in UAE North, its HD voice and transcription scopes are Global, and its LLM component has a separate scope. Therefore, Voice Live does not meet a UAE-only requirement in that configuration.^6^

The standalone Azure OpenAI Realtime API documentation describes realtime models as Global deployments, so it likewise does not establish UAE-only processing.^7^

Azure Speech includes UAE Arabic voices ar-AE-FatimaNeural and ar-AE-HamdanNeural, plus Lebanese, Jordanian, Saudi, Kuwaiti, Qatari, Omani, and other Arabic locales.^8^

UAE North supports hosting and synthesis for a custom voice but not training. A professional voice can be trained in a supported training region and copied to UAE North.^9^ That workflow necessarily sends training data outside the UAE, so it does not satisfy a rule requiring all processing to remain inside the country. Custom-voice acknowledgment data and models can also be retained for service-integrity purposes; personal voice does not support customer-managed storage.^10^

    Compliance and contractual review

    • Azure’s UAE DESC certification applies to UAE North and UAE Central, with audit material available through the Service Trust Portal. It does not by itself prove compliance with DHA or the federal health-data law for your particular architecture.^11^
    • Compliance Manager includes assessments for UAE Federal Law No. 2 of 2019 and the UAE Data Privacy Law.^12^
    • Obtain the Microsoft DPA at https://aka.ms/DPA and the Online Services Subprocessor List at https://aka.ms/subprocessor.^13^
    • Because differential-diagnosis and medication recommendations may constitute medical-device functionality, obtain UAE regulatory advice; Microsoft states that its online services are not supplied as medical devices or substitutes for professional judgment.^14^

Ask your Microsoft account team or open an Azure support request for a written statement covering the exact model/SKU, inference boundary, abuse monitoring, operational logs, support personnel access, subprocessors, backups/failover, and every Speech/custom-voice data flow. Have UAE healthcare counsel incorporate the required commitments into the applicable contract or order documents.
