lost registration form from February 2025

I cannot find the registration form that I created and have used since February 2025. It has all the feedback from the training sessions that I deliver and need for evaluation purposes.

I have tried all of the self help guides, Q&A but cannot find it anywhere.

can you please help me.

I can see that you’re unable to find the form and its training feedback.

Here are what you can do:

Please sign in to Microsoft Forms using the exact Microsoft account originally used to create the form, then check All forms and Deleted forms.
If it appears under Deleted forms, select More options > Restore. Deleted forms remain there unless they were manually removed from that area; permanently removed forms and their responses cannot be restored through Forms.

As an additional check, search your browser history, emails, or messages for the original form link. Opening that link while signed in may help confirm whether the form still exists under a different account.
