    Regarding the details you shared, the Session ID and Correlation ID are internal Microsoft tracking codes used for diagnostics. Unfortunately, these cannot be used to open or retrieve the form directly.

    To better understand what’s happening, could you please share a bit more information?

    • Are you opening the form from a direct link, email, or Microsoft Teams?
    • What exactly happens when you try to open it (for example: “This form does not exist”, “Access denied”, or “Something went wrong”)?
    • Is this your own form that you created, or a form that someone shared with you?
    • Do you know when the form was created and whether it has a due date or expiration set?
    • Are you signed in with a work/school account or a personal Microsoft account?

    In the meantime, you can try opening the form in a private/incognito window:

    • Press Ctrl + Shift + N (Chrome/Edge)
    • Go to forms.office.com
    • Sign in with the same account used to create or access the form
    • Try opening the form again

    This helps rule out issues related to cached data or account session conflicts.

    You may also try a different browser or device to see if the behavior changes.
