This behavior is usually caused by a sync issue between Legacy Outlook and Apple Calendar, where declined events are not properly updated on the Exchange server.
May I know if the suggestions from Q&A Assist worked for you? In many cases, removing and re‑adding the Outlook/Exchange account on the Apple device resolves the sync issue.
If the issue persists, you can try the following additional steps:
1.Refresh from Calendar App
-Open Calendar on macOS > View > Refresh Calendars (Command + R).
2.Open Calendar on macOS > Go to Calendar > Settings > Make sure Enable this account is checked for your Exchange account.
3.Clear cached data
Please make sure to back up the Calendar Cache file before deleting or moving it.
-Quit the Calendar app.
-Open Finder and navigate to: ~/Library/Calendars/Calendar Cache
-Copy the Calendar Cache file to a safe backup location.
-Delete or move the original Calendar Cache file.
-Reopen Calendar and allow it to re-sync with Exchange.

4.If the above steps do not work, try declining the meeting directly in Outlook Web App. This ensures the server is updated correctly, and the decline propagates properly to Apple Calendar.
