I created an App install in Intune for Acrobat and assigned a group to the App which is listed as "Required". The device in the group is Hybrid AD joined and the onprem SCCM server is set to allow Intune to be the primary source of truth for Windows Updates, Device configs etc... No matter how many reboots or sync's I do the App never installs.

What is the compliance status for the app in Intune? Is the issue with only one app or all apps? Are the clients healthy for co-management? Have you also moved the Device Apps workload to Intune?
Compliance status is Unknown for the App, but the device is 100% compliant in Intune. As for the App it was 100% built in Intune and nothing came from SCCM.
I tried to push the Company Portal to my test device and that is stuck in an unknown install status. I am wondering if there are issues with the device.
The device I am pushing the App to is online yet will not install any L.O.B App or anything from the Microsoft Store like the Company Portal. I do see the device checking in though. We do have a onprem SCCM server but this is set to have Intune control everything thoughts?

I am not sure why my previous comment was deleted? Anyway I did find the issue, it was SCCM, once I uninstall the agent the App installed.
Delete LastFullReportTimeUtc from HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\IntuneManagementExtension\Win32Apps\Reporting{UserId} and then performed a sync
