Why isn't my table of contents defaulting every item to page 1?

My table of contents is defaulting all the items to be displayed as page 1.

    Does updating the TOC make a difference?

    A sample document may help, as other contributors have already suggested. Special considerations may apply if you are combining several documents, for example using RD fields.
